502-585-5230 KENTUCKY REHABILITATION ASSOCIATION AWARD
CATEGORIES 1. BEN F. COFFMAN AWARD: For outstanding service to the rehabilitation of individuals with disabilities. This award is for any employee of the federal, stated, or local governments, or private agencies. The winner must be a member of KRA and shown to have given exceptional service
to citizens with disabling conditions. 2. PERSONAL ACHIEVEMENT AWARD/PROFESSIONAL ACHIEVEMENT AWARD: This award is to recognize a consumer of rehabilitation services who has demonstrated exceptional determination and motivation over substantial impediments to employment resulting from a significant disability, and who has successfully completed a rehabilitation program and become gainfully employed. The Professional Achievement Award is to recognize the outstanding working partnership between the nominee and the professional who worked with the nominee to help them obtain their goals. The Professional must be a KRA member. The recipients of these awards will be the KRA nominees for
the Southeast Region National Rehabilitation Association (SERNRA). 3. WENDELL TAYLOR MEMORIAL AWARD: This award is given to an individual or group, whose dedication to rehabilitation motivates them to work without any or little financial reimbursement. The activities may be an independent operation of in cooperation with professional rehabilitation workers.
Nominations do not have to be a KRA member. 4. GERALD N. WILLIAMS ACCESSIBLITY AWARD: This award is presented to an individual business or organization that has exercised leadership in removing environmental and legal barriers and in overcoming
discrimination against persons with disabilities. 5. CHARLES MCDOWELL EDUCATION & ADVOCACY AWARD: This award is presented to an advocate for persons with disabilities who exercised creative and effective leadership on behalf of rehabilitation programs; and/or who has made a major contribution to the rehabilitation of persons with
disabilities through actions that increased rehabilitation opportunities for
persons with disabilities. 6. OUTSTANDING EMPLOYER OF PERSONS WITH DISABILITIES AWARD: This award is made to employers across the state that has contributed to the rehabilitation of individuals with disabilities through
employment. 7. KRA STAFF AWARD: This award is given to a full time clerical employee of a public or private organization or agency who has displayed outstanding service and contributions to persons with disabilities.

Nomination Form: MARGARET T. WILSON SERVICE AWARD Person nominating must be a member of KRA. Nominees must be a member of KARSS. This award will be given to a full-time employee whose dedication and service to serving individuals with disabilities far exceeded normal requirements of the job.
