Psychological Fee Schedule Amended to reflect minimal rate increases for P9012 and P9013 Individual Therapy and to add guidelines for purchase of services from Clinical Nurse Specialists, Advanced Registered Nurse Practitioners with Masters in Mental Health, Licensed Marriage and Family Therapists, Licensed Professional Clinical Counselors, and Certified Professional Clinical Counselors. Current Vocational Rehabilitation Federal Regulations indicate that diagnosis and treatment for mental and emotional disorders must be provided by qualified personnel in accordance with State licensure laws and regulations. A certified psychologist with autonomous functioning requires no supervision and may direct bill the agency for assessments and treatment. A certified psychologist and a licensed psychological associate must be supervised by a licensed psychologist. Assessments, treatment plans, and progress reports must be countersigned by the supervising psychologist. A certified psychologist may direct bill this agency. A licensed psychological associate may not direct bill, rather the supervising licensed psychologist must direct bill for DVR payment. A licensed clinical social
worker (LCSW) does not require supervision and may direct bill this agency. Clinical nurse specialists and

Exception to Yearly Filing of Free Application for Federal Student Aid (FAFSA)
If the consumer files the FAFSA and is not eligible for any
need-based financial aid (including grants or loans),
a new SAR is not required in subsequent training years if there has been no
substantive change in family
income, dependency status, or school of attendance. The initial SAR should be
used to complete the
Worksheet.

Consumers who file the FAFSA on the Web will not receive a hard copy SAR in the mail. A copy of the SAR can only be printed from the FAFSA web site.

RE: Remediation Services through Project Mentor at Murray State University
Project Mentor offers enhanced remediation services to
students with a diagnosed learning disability, attention
deficit disorder, or acquired brain injury. Additional information about the
program may be found
on Page Three of this Memorandum.
Consumers/students are to contact Project Mentor to request
an application. The Department of
Vocational Rehabilitation (DVR) will not cover the $25.00 Application Fee that
the student must return
with the completed application.
Effective with Summer, 2002, Murray has initiated changes to the billing and
payment process for Project Mentor. A flat rate per semester will be charged for
mentoring services. The rate will vary based on the number of hours needed;
i.e., one hour per week up to five hours per week. The student must file a
mentor request form with the Services for Students with Learning Disabilities (SSLD)
Office each semester contracting for the number of hours needed. Murray will
bill that flat rate at the beginning of each semester. DFB/DVR is to invoice
upon receipt of the bill. No refunds will be given. Failure to utilize the
requested hours could affect the amount of mentoring provided for subsequent
semesters.

Murray State University Project Mentor
Project Mentor is an academic support unit of the Learning
Center that offers enhanced services
to students with a diagnosed learning disability, attention deficit disorder, or
traumatic brain injury.
This is a comprehensive program that focuses on the development of strategies
and skills to build
upon strengths and circumvent deficits. Students voluntarily access the services
provided through
Project Mentor. They receive individualized assistance with learning effective
strategies for
organizing and studying course-related materials and assignments. Accommodations
such as
taped textbooks and extended test time are also available based on each student’s
particular
needs and specific disability.
The Project Mentor staff who provide direct services include a Director, an Assistant Director, and Academic Mentors (graduate students). Each student is assigned to work with one of the Academic Mentors on a regular weekly basis (usually 3 to 4 times per week). This work may involve any of the following: writing skills, reading strategies, note-taking skills, test-taking strategies, organization/time management skills, self-advocacy skills, social skills, career-related issues, and referrals to other campus resources. These services are offered in addition to the accommodations required by law.

This SERVICE FEE MEMORANDUM replaces SFM
MS-JT-00-01-10. The Windows 2000 Microsoft
Certified Systems Engineer (MCSE) Program offered at A Technological Advantage
(ATA) in
Louisville has been updated. A DFB/DVR allowable tuition rate of $7,495.00 will
be accepted as
tuition payment in full.
The maximum DVR assistance that any consumer can receive with
the Program (Tuition)
Fee is 75% of the allowable tuition rate. The consumer will be responsible for
the remaining
payment up to the allowable rate.
Pay Authorizations
Issue two pay authorizations. Issue the first pay authorization prior to enrollment in the A+ Certification course. ATA may bill for this first payment when the consumer has completed the A+ certification. If the consumer has shown satisfactory progress in the first three courses as demonstrated by passing the post-class assessments, issue the second pay authorization prior to enrollment in the fourth course. ATA may bill for this payment when the consumer has completed the fourth MCSE Course.

RE: 2002 Summer School Tuition
The highest undergraduate tuition rate at a state-supported institution in Kentucky for the May Intersession and for Summer School, 2002, is $162.00 (not $171.00) per credit hour. Tuition at $162.00 per credit hour is therefore the maximum DVR rate that may be authorized for undergraduate courses at any private or out-of-state school.
NOTE: If you have already issued the Tuition Expenditure Worksheet or the Pay Authorization to a private or out-of-state school for $171.00 per credit hour, this will be acceptable.
The highest graduate tuition rate at a state-supported institution in Kentucky for the May Intersession and for Summer School, 2002, is $235.00 (not $227.00) per credit hour. Tuition at $235.00 per credit hour is therefore the maximum DVR rate that may be authorized for graduate courses at any private or out-of-state school.
Tuition at the University of Louisville will be $162.00 per undergraduate credit hour and $235.00 per graduate credit hour with no Student Building Fee. University of Louisville Pay Authorizations will need to be amended to these rates. Tuition at the University of Kentucky for an undergraduate course will be $148.90 per credit hour and tuition for a graduate course will be $212.90 per credit hour.

The Kentucky Department of Vocational Rehabilitation (DVR) can provide assistance with the costs of computer technology training offered through Manufacturing Technology Transfer Center (MTTC) under its current status as a public/private partnership operated by Innovative Productivity, Inc., for the United States Navy Manufacturing Technology Program. MTTC is located at 119 Rochester Drive in Louisville.

RE: Educational Support Services Fee Schedule
Effective November 1, 2001, this updated DFB/DVR Educational Support Services Fee Schedule is replacing the August 24, 2000, Fee Schedule, DVR Service Fee Memorandum MS-JT-99-00-37. The Educational Support Services Fee Schedule is in the Case Management System and is to be used by both agencies.
Rates for 00072 Educational Aide, Reader, Notetaker, Page Turner, and Typist have been increased up to $7.00 per hour. The semester totals for Reader Services have also been increased.
To be reimbursed at $10.00 per hour for tutoring, the individual must have a Bachelor’s Degree in, or a minimum of fifteen semester hours in, the specific subject matter. When the tutor meets the educational requirements listed in previous sentence and can communicate with the consumer in his/her native language with the service of an interpreter not required (interpreting for individual who is deaf included), the hourly rate may be increased to $12.50 per hour. No increase in hourly rate will be granted for collective tutoring and/or educational aide.
The following Semester maximums that apply to Reader Services have been increased:

Credit hours at Decker College of Business Technology may not always be evenly divided between the two quarters of a six-month program and among the three quarters for a nine-month program. It is therefore permissible to increase or to decrease the tuition allowable per quarter, as long as the total tuition assistance is not exceeded.
INDIVIDUAL CLASSES - TWELVE-QUARTER CREDIT HOURS OR LESS
Partial tuition assistance at 75% or less of $171.00 per
credit hour will be provided for remedial keyboarding classes and individual
classes. The DVR consumer will be responsible for the remainder of tuition
charges for individual classes.
Up to a total of twelve-quarter credit hours of remedial keyboarding and/or individual classes may be covered. Should the consumer continue to enroll in individual classes in excess of twelve-quarter credit hours, DVR assistance with the cost of these additional classes will not be provided.
TRANSFER OF CREDIT HOURS
Should the consumer choose to transfer into a certificate program after initially taking remedial keyboarding classes and/or individual classes, DVR tuition assistance up to the maximum partial allowable for the particular certificate program will be provided. Should the consumer choose to apply credits earned in the six- and nine-month certificate to an associate degree program, DVR tuition assistance up to the maximum partial allowable for the associate degree program will be provided.

Partial tuition assistance at 75% or less of $142.00 per credit hour will be provided for remedial keyboarding classes and individual classes. The DVR consumer will be responsible for the remainder of tuition charges for individual classes.
Up to a total of twelve-quarter credit hours of remedial keyboarding and/or individual classes may be covered. Should the consumer continue to enroll in individual classes in excess of twelve-quarter credit hours, DVR assistance with the cost of these additional classes will not be provided.
TRANSFER OF CREDIT HOURS
Should the consumer choose to transfer into a certificate program after initially taking remedial keyboarding classes and/or individual classes, DVR tuition assistance up to the maximum partial allowable for the particular certificate program will be provided. Should the consumer choose to apply credits earned in the six- and nine-month certificate to an associate degree program, DVR tuition assistance up to the maximum partial allowable for the associate degree program will be provided.

Network + is a testing program sponsored by the Computing Technology Industry Association (CompTIA) that certifies the knowledge of networking professionals with 18 to 24 months experience in the Information Technology industry. This test is administered by NCS/VUE and Prometric. Earning the Network + certification means that the individual possesses the knowledge needed to configure and install the TCP/IP client. This exam covers a wide range of vendor and product neutral networking technologies that can also serve as a prerequisite for vendor-specific Information Technology certifications. This course is 40 clock hours in length.
A+ CERTIFICATION (Provided by Panurgy)
A+ Certification is a testing program sponsored by CompTIA that certifies the competency of entry-level (6 months experience) Service Technicians in the computer industry. Earning A+ Certification means that the individual possesses the knowledge, skills, and customer relations skills essential for a successful entry-level (6 months experience) computer Service Technician, as defined by experts from companies across the industry. The test covers a broad range of hardware and software technologies, but are not related to any vendor-specific products. To become certified, you must pass two parts, Core and Microsoft DOS/Windows module portion. When both the Core and the DOS/Windows portions are passed within 90 calendar days, the individual receives the A+ designation. This course is 40 clock hours in length.
PROCEDURES
One DVR pay authorization is to be issued prior to enrollment in the A+ Certification or Network + Program. Panurgy will bill for payment when the consumer has completed the 40 clock hour program.
The measure of satisfactory progress will be an in-house pre-certification testing process as well as CBT based augmentation of the Instructor Led portion for either the A+ Certification or Network + Program.

LAPTOP COMPUTERS
At Home Programs are available on laptop computers provided by Computer Career Centers (CCC) at a charge of $1,365.00. If a consumer has a laptop that fits the needed specifications, CCC will arrange for this laptop to be loaded with the training materials. When DVR provides assistance with the purchase of a laptop, $1,365.00 can be paid to CCC.
PROCEDURES
Separate DVR pay documents will be issued per course to cover tuition and the Microsoft Certification Examination. CCC will bill once the consumer has enrolled in the course.
When the consumer has shown satisfactory progress in the first course, the pay document for the second course will be issued. As long as satisfactory progress being shown by the consumer, this procedure will be repeated until all courses completed.
This agency will consider passing the CCC exit examination for each course included in a program a satisfactory measure of progress. Test results are to be provided to the DVR counselor.
When a laptop computer is purchased from CCC, a separate DVR pay document will be issued at the time that the computer is ordered. Authorize from regular caseload budget not the assistive technology budget. CCC may then bill when the computer has been delivered to the consumer.

If a student receives enough financial aid to pay the bill in full, the student will receive a waiver until the financial aid funds are released and no payment is required by the August 14 deadline. RE: 2000 Summer School Tuition
The highest undergraduate tuition rate at a state-supported institution in Kentucky for the May Intersession and for Summer School, 2000, is $133.00 per credit hour at the University of Louisville. Tuition at $133.00 per credit hour is therefore the maximum DVR rate that may be authorized for undergraduate courses for 2000 Summer School Sessions.
In addition to tuition at $133.00 per undergraduate credit hour, the University of Louisville will also charge a Student Building Fee at $15.00 for 0-6 hours and at $30.00 for 7 credit hours or more for Summer School, 2000.
The highest graduate tuition rate at a state-supported institution in Kentucky for the May Intersession and for Summer School, 2000, is $209.50 per credit hour at the University of Louisville. Tuition at $209.50 per credit hour is therefore the maximum DVR rate that may be authorized for graduate courses for 2000 Summer School Sessions.
Summer School tuition at the University of Kentucky for an undergraduate course will be $130.00 per credit hour. Summer School tuition at the University of Kentucky for a graduate course is $188.00 per credit hour.

Microsoft Certified Systems Engineer Program (MCSE)
A Technological Advantage, Inc., Louisville
A Technological Advantage, Inc., in Louisville has agreed to accept the Department of Vocational Rehabilitation
(DVR) allowable tuition and fees as payment in full for DVR-sponsored consumers enrolled in its Microsoft
Certified Systems Engineer (MCSE) Program, Microsoft Certified Systems Engineer (MCSE) and A+ Technology
Programs, and Microsoft Certified Solutions Developer (MCSD) Program.
OUTCOME MEASUREMENTS
Passing the A Technological Advantage (ATA) post-class assessments rather than passing the Microsoft
Certification Examinations will be considered a satisfactory assessment of outcome for the MCSD and MCSE
courses. Passing the Microsoft A+ Certification Examination will still be considered the outcome assessment for
the A+ Certification course. DVR will continue to provide coverage for all MCSD and MCSE Microsoft
Certification Examinations. And consumers will still be expected to pass all MCSD or MCSE Microsoft
Certification Examinations, but will no longer be required to pass the Certification Examination prior to
enrollment in the next MCSD or MCSE course.
PAYMENT PROCESS
Microsoft Certified Systems Engineer (MCSE) and A+ Certification Programs
Three DVR pay documents will be issued. The first pay document will be issued prior to enrollment in the
A+ Certification course. ATA may bill for this first payment when the consumer has completed the A+ Certification
course. If the consumer has shown satisfactory progress in this course as demonstrated by passing the
Microsoft A+ Certification Examination(s), the second pay document will be issued prior to enrollment in
the first MCSE course. ATA may then bill for this payment when the consumer has completed the first MCSE
course. If the consumer has shown satisfactory progress in the first three MCSE courses as demonstrated by

Credit hours at Computer Education Services may not always be evenly divided between the two quarters of a

The Department of Vocational Rehabilitation (DVR) will cover New Horizons proposed package pricing of $5,595 for the Microsoft Certified Solution Developer (MCSD) Program which includes one prerequisite course (#1303) and the four Instructor Led Training (ILT) courses. In addition to the $5,595 fee for the five-course program, DVR will allow an additional $1,500 for the Computer Based Training (CBT) lab that includes the "Test Pass Guarantee." RE: Training Updates - Filing Electronically for Financial Aid
You may wish to encourage consumers who have not filed their 1999-2000 Free Application for Federal Student Aid (FAFSA) to do so electronically. Consumers filing electronically should receive the Student Aid Report (SAR) more quickly.
There are three ways to file electronically: FAFSA on the Web, FAFSA Express, and through the Financial Aid Office at the Consumer’s School. Details on these three methods are on Page 2 of Funding Your Education 1999-2000. One copy of this publication was sent to each specialist earlier this year (blue cover with green print).
I did consult with a financial aid counselor as to which of the above electronic methods would be the best for DVR consumers at this point in time. She recommended filing electronically through the Financial Aid Office. She did caution that a consumer filing FAFSA on the Web needs to have a printer attached to the computer for the signature page; otherwise, it will take about the same time to receive the SAR as mailing a paper copy.
The inside front cover page of Funding Your Education 1999-2000 also has some frequently requested telephone numbers and web sites for FAFSA related issues.

RE: Training Update – Financial Aid Notification (FAN)
If some of your consumers have been in receipt of their Student Aid Reports (SAR’s) for several weeks, but indicate that they still have not received their Financial Aid Notifications (FAN’s), it is possible that these consumers will never receive FAN’s or will receive late.
Some of the technical colleges do not send out FAN’s until after the beginning of the Fall Semester. For returning students, I would suggest that yiou use their 1998-1999 School Year FAN’s to complete the Overaward section of the form. You can then complete the Tuition Participation Worksheet section with information from the 1999-2000 SAR’s.
The University of Kentucky Student Financial Aid Office (SFAO) does not send a FAN to any student who is not eligible for need based aid and who does not wish to apply for a loan. To prevent overawards, the University of Kentucky SFAO also will not send FAN’s to students who received VR support last school year and are only eligible for loans until after they receive your 1999-2000 DVR Tuition Participation and Overaward Worksheet.
If you are in receipt of a new student’s SAR but not a FAN, you might check Page 1 of the SAR to see if the consumer is eligible for a Pell Grant. If eligible, then the student should receive a FAN. If not eligible for a Pell rant, it is possible that the consumer is not eligible for any need based aid and will not receive a FAN. But you will need to confirm this with the SFAO.
I encourage you to contact the Student Financial Aid Office on new students who have received SAR’s but not FAN’s. You might consider sending a short form letter which would include the student’s name and social security number with a YES or NO response to be checked by the SFAO. YES – The student is eligible for financial aid and has received or will be receiving a FAN. NO – The student is not eligible for financial aid and will not be receiving a FAN. For any YES responses who have not been sent a FAN, you might ask for the amount of the Total Award and a Breakdown. I don’t promise that you will get information on amounts of awards, but I do feel that you will get responses as to the eligibility for financial aid. I do have email addresses for some of the SFAO which I could share with you if you prefer to communicate via email.
As you work through this new system for the first time with both new and returning students, please share with me both the things that seem to work well and the things that don’t. And don’t hesitate to call me for assistance.

RE: New Horizons Computer Learning Center of Lexington
New Horizons Computer Learning Center of Lexington has agreed to accept a total tuition and fees charge of
$4,435.00 for its Microsoft Certified Systems Engineer (MCSE) program for consumers who enter their first MCSE
class after January 1, 1999. $4,435.00 is the highest tuition and fees charged by a state institution. New Horizons has
agreed not to charge consumers being assisted by this agency the difference between its actual charge for the MCSE
program and the $4,435.00. For consumers who entered their first MCSE class prior to January 1, 1999, tuition and
fees charge as outlined in Service Fee Memorandum MS-98-99-6 should be allowed for the completion of the program.
The Department of Vocational Rehabilitation will no longer pay $100.00 per course practice examination. Consumers
can download the practice examinations from the Microsoft Web Site.
The Department of Vocational Rehabilitation will continue to pay $100.00 per Microsoft certification examination for
the six included in the MCSE program.
New Horizons continues to review with the prospective student/consumer, Beginning Windows 95 and
Intermediate/Advanced Windows 95. If the consumer does not have a thorough understanding of these two courses,
the consumer will be offered both of these courses at no cost.
AUTHORIZATIONS
Two authorizations are to be issued. The first authorization is to cover the first three courses and certification
examinations. After the consumer has completed the first course, New Horizons will bill and this agency will then
invoice for payment. The second authorization covering the last three courses and certification examinations will not
be issued until the consumer has passed the first three Microsoft certification examinations. After the consumer has
completed the fourth course, New Horizons will issue its second bill and this agency will invoice for payment.

A supply of the 1999-2000 Free Applications for Federal Student Aid (FAFSA’s) and State Grant Information Sheets will be mailed to each vocational rehabilitation office by mid-December, 1998. The FAFSA Cover Page does not contain information about Kentucky Higher Education Assistance Authority (KHEAA) State Grants. Please collate a State Grant Information Sheet with the FAFSA before giving to a consumer.
First-time applicants must file the Free Application for Federal Student Aid (FAFSA). Renewal applicants should file the Renewal FAFSA that they received in the mail.
Consumers must also file for state grants and can do so on the FAFSA or Renewal FAFSA. KHEAA administers the state grants,, i.e., College Access Program (CAP) for public schools, Kentucky /tuition Grant (KTG) for private schools, and Teacher Scholarship (TS).
The KHEAA priority mailing deadline is March 15, 1999. Please encourage consumers to mail the FAFSA’s or Renewal FAFSA’s in February or early March, 1999. However, consumers must not file prior to January 1, 1999. State grants and campus-based federal aid are awarded on a first-come, first-served basis to eligible students until the funds allocated for the 1999-2000 School year are depleted.

RE: Health Clinic Fee, Prestonsburg Community College
Beginning with the Spring Semester, 1995, all fulltime students at Prestonsburg Community College (PCC), Main Campus and Pikeville Campus, will be charged a mandatory $82.50 Health Clinic Fee.
Any student receiving the maximum Pell Grant award of $1,150.00 for the Spring Semester, 1995 will receive a scholarship from PCC to cover the Health Clinic Fee. DVR coverage of the Health Clinic Fee should therefore not be provided for any client receiving a maximum Pell Grant award. If you have already issued an EP-8 for the Spring Semester with the Health Clinic Fee included for a client receiving the PCC scholarship, you should cancel the Health Clinic Fee immediately.
DVR assistance with the Health Clinic Fee may be provided for any student not receiving the PCC scholarship if need is evident on the DVR-4C. When DVR assistance is provided with the Health Clinic Fee, Part D of the DVR-4C should be amended and a photocopy sent to the Financial Aid Office.

RE: Tuition Deposit at Western Kentucky University
For the past three school years, Western Kentucky University has required a $100 Advance Tuition Deposit to guarantee preregistered classes. The Tuition Deposit must be paid each semester. The Deposit will not be waived for DVR clients nor will a DVR EP-8 covering tuition be acceptable in lieu of the payment.
If full DVR tuition assistance is then provided, the student may later be refunded the Tuition Deposit or may request that the $100 be applied toward the upcoming semester’s Tuition Deposit. For those students with limited resources, I would encourage the latter.

RE: Flight Training
Flight training may only be purchased from providers certified by the Federal Aviation Administration (FAA) to train under FAR Part 141. Flight training may not be purchased from providers with FAA certification to train under FAR Part 61. FAR Part 141 certification establishes the provider as a school. A provider with FAR Part 61 certification is typically a business offering private flight lessons. Only providers certified under FAR Part 141 meet this agency’s regulation that training may only be purchased from appropriately accredited or licensed schools.
There are currently two schools in Kentucky which offer flight training. These schools are Aero Tech Inc., a private school in Lexington, and Eastern Kentucky University, a state school in Richmond.
Since similar vocational preparation is available through a state institution, tuition and fees assistance to Aero Tech, any other private school, or out-of-state school offering flight training may not exceed that allowed to the highest state institution. As aviation fees vary according to the level of licensure being pursued, please contact me for allowable tuition and fees assistance when training is not being pursued through Eastern Kentucky University.
RE: Kentucky Tech/Jefferson State Campus Carpentry: Qualified Framer Program

Tuition for the new twenty-eight (28) week Carpentry: Qualified Framer Training Program being offered through KY Tech Jefferson State Vocational Technical School should be authorized on EP-8-3’s. Two EP-8-3’s for tuition assistance should be issued – one at the beginning of the course and the second around the fourteenth week of training.

For the 1993-94 School Year, the Financial Aid Office at Berea College has included the Cost of Education Fee (Tuition) in the School Budget in Part C on the DVR 4-C.
In previous years, the Cost of Education Fee has not been included on the DVR 4-C and the Department of Vocational Rehabilitation (DVR) has not provided assistance with this Fee since Berea guarantees that the Cost of Education Fee will be covered and not charged back to the student or family.
For the 1993-94 School Year, please continue to provide assistance with the costs for which students are responsible; namely, Health and Activities Fees, Books and Supplies, Room, and Board. Do not provide assistance with the Cost of Education Fee (Tuition).
The ramifications of DVR assistance being switched from those costs for which students are responsible to the cost of tuition for which the student cannot be charged needs to be further explored.

For the first time, students at the University of Kentucky (UK) Lexington Campus and Lexington Community College who wish to pre-register for the 1993 Fall Semester will be required to pay a $50 advance Registration Fee. Pre-registration dates are March 30-April 13 for UK Lexington Campus and April 5-16 for Lexington Community College.
The Department of Vocational Rehabilitation (DVR) cannot cover this fee as payment of a fall semester fee cannot be made prior to the actual enrollment date. This fee will not be waived for DVR clients. Clients will be responsible for payment of this fee.
No other community college in the UK System will be charging this advance Registration Fee.

Tuition Deposit – Western Kentucky University, Bowling Green
To guarantee preregistered classes for Fall Semester, 1992, a student must make a $100 tuition deposit to Western Kentucky University by July 22. This deposit will not be waived for DVR clients nor will a DVR EP-8 covering Fall Semester tuition be acceptable in lieu of the payment.
Graduate/Professional School
DVR assistance with tuition and fees for training beyond the Baccalaureate level is based on economic need as determined by the economic needs test on the DVR-2. It is not necessary to complete the DVR-4C for graduate or professional school.
Client Access Program (CAP)/State Student Incentive Grant
Two state grant programs, the Client Access program (CAP) and the State Student Incentive Grant Program, have been merged. The program will not be known as CAP. Applicants must be enrolled a minimum of six hours to receive CAP.
JOBS Program
Due to limited funds, the Department for Social Insurance has implemented changes in the JOBS Program effective July 1, 1992.
JOBS funds cannot be utilized for tuition or books for postsecondary education. Course fee payments such as lab fees, materials, or activity fees attached to a class will not be covered. JOBS will continue to cover entrance application fees and financial aid application fees.
JOBS funds cannot be utilized for support services including parenting classes, life skills classes, food and nutrition classes.
JOBS will no longer authorize pre-employment expenses.
Kentucky Polytechnic Institute, Louisville
This school has closed. Students have been transferred to Louisville Technical Institute.
Brannon Business Institute, Paducah
This school has been suspended from participation in the Kentucky Higher Education Assistance Authority Guaranteed Student Loan Programs.

RE: University of Louisville Application Fee
The University of Louisville is charging a new $25 Application Fee to any first-time applicant. This agency will not routinely cover the Application Fee; rather clients will be expected to pay the Fee or to apply for one of the waivers listed in the next paragraph.
The University will consider waiving the Application Fee if an individual is eligible under one of the following:
ACT/SAT Waiver (high school student), or
Financial Disability Waiver (AFDC, SSI, SSDI, JTPA, Medicaid, Food Stamps Program, JTPA recipient).
The Application Fee will not automatically be waived for DVR clients as not all DVR clients evidence financial need. It will be the responsibility of the client to request the waiver through the Admissions Office.
If covered by DVR, the Fee should be included on the same authorization as Fall, Spring, or Summer Term assistance and must not be invoiced until the client is actually enrolled in his/her first term. A copy of the authorization should be mailed to Janice Prestigiacomo in the Admissions Office.

Other Agency approved postsecondary training programs for individuals who are deaf which offer interpreter services, notetaking services, and tutoring services, but are not recognized by the U. S. Congress as national programs, may be utilized for clients. But the total cost of attendance must not exceed the total cost of provision of tuition and fees, interpreter services, notetaking services, and tutoring services at the highest state-supported Kentucky institution that offers similar vocational preparation. (See Counselor Manual, Section 5017, 3.)
For the 1994-95 School year, this total cost is $22,766. This amount does reflect inclusion of the following budget items: tuition and fees, room and board, books and supplies, personal expenses, transportation, interpreting services, notetaking services, and tutoring services.
Comparable benefits and the DVR-4C guidelines must be applied. Kentucky DVR allowable hourly rates for interpreter, tutoring, and notetaking services must not be exceeded.

SUBJECT: OVR Assistance for Post-Secondary Education to Clients in Default on Student Loans
If a client who has defaulted on a student loan has made no attempts to make arrangements to repay the loan or has simply decided not to repay the loan even though financially able to do so, the VR program is prohibited from paying the client’s college costs.
On the other hand, a client who has limited or no financial resources available and cannot work out a satisfactory repayment agreement with the lender may be considered for VR assistance. Under circumstances where a responsible repayment effort has been made, it may be concluded that maximum effort has been made to secure financial aid and that such similar benefits are not available.
A client who has defaulted on a student loan should proceed to clear his/her default status.
For the Perkins Loan program, a client is no longer in default if:
(1) all past-due amounts have been repaid, cancelled, or deferred;
(2) the client’s loan has been discharged in bankruptcy; or
(3) the client has entered into a new repayment agreement for the loan.
For the Stafford (Guaranteed Student Loan) program, a client may no longer be in default status if:
(1) the holder of the loan certifies for the purpose of reinstating Title IV eligibility that the borrower has made satisfactory arrangements to repay the defaulted loan; or
(2) the loan is discharged in bankruptcy.
