RE: Prosthetic Services Fee Schedule

An updated Prosthetic Services Fee Schedule was completed on the Case Management System effective July 1, 1998. We failed to issue the accompanying Service Fee Memorandum at that time.

Medicare HCPCS Codes and Descriptions for Prosthetic Services are being utilized on this Schedule.

For any "By Report" item, obtain item description, charge, and justification. Then contact the Central Office for approval to purchase.
