Please destroy SERVICE FEE MEMORANDUM MS-97-98-36 (September 23, 1998) and SERVICE FEE MEMORANDUM MS-97-98-37 (September 23, 1998) and replace with this current MEMO. The fee schedules for Purchase of Existing Medical Information and for Examinations and Consultations are being combined. This MEMO only reflects code changes, but no rate changes, and is effective immediately.
In the routine development of rehabilitation cases, medical information already on file in a hospital or physician's office may be helpful in documentation and evaluation procedures. When the information is felt to be essential but will not be provided without reimbursement, procedure 00039, 00040, and 00041 may be authorized.
Written reports must be furnished. Special medical forms may be used in lieu of a narrative report.
Physical Therapy Evaluation and Occupational Therapy Evaluation procedure codes 00045 and 00046 have been deleted. Refer to Relative Value Schedule (RVS), Medicine Sction codes, 97001, 97002, 97003, and 97004 for these evaluation procedures.
Refer to RVS, Medicine Section, codes 98940, 98941, and 98942 for allowable rates for Chiropractic procedures and to SERVICE FEE MEMORANDUM MS 94-95-12 (Updated and Addendum), November 29, 1995, for guidelines for Chiropractic services.

In the routine development of rehabilitation cases, medical information already on file in a hospital or physician’s office may be helpful in the documentation and evaluation procedures. When the information is felt to be essential but will not be provided without reimbursement, the following schedule should be considered as maximum amounts to be authorized:
MS-93-94-12 DATE: December 16, 1993 RE: Purchase of Medical Records at University Hospital University Hospital, Lexington, now contracts with the Smart Corporation for the photocopying of medical records. Authorizations should therefore be issued to Smart Corporation when copies of medical records being requested from University Hospital. |
