RE:      SSA to Community Rehabilitation Programs

  An Outcome Based Fee of $5,000.00 per individual may be allowed to a Community Rehabilitation Program  (CRP) for the provision of Individual Follow-Up Services to assist an individual who is a Social Security Disability Insurance recipient to maintain continuous employment for nine to twelve months at the current level of Substantial Gainful Activity (SGA) level as defined by the Social Security Administration.  Follow-up Services must include Essential Support Services and Job Retention Services as defined in this memorandum. 

This fee only applies to CRP service providers.

The CRP will provide the follow-up services needed to support and maintain an individual in employment until the individual maintains SGA for nine out of twelve months.  Support services will vary, depending on the individual’s situation and/or needs.

  Essential Support Services:

To satisfy the requirement for payment two essential support services must be provided in conjunction with job retention services.

 

Monthly Assistance - In reporting correct income to SSA to avoid overpayment of benefits and prevent job abandonment.

Counseling - To retain Medicare and/or Medicaid benefits by utilizing SSA work incentives. For example: extended Medicaid benefits under SSA 1619B. 

Education and Training - On integrating Medicare and Medicaid benefits with employer benefits. 

Counseling on SSA Work Incentives - To identify specific incentives that benefit the consumer.

Utilization of SSA PASS Plan and/or Other Resources - In coordination with employer educational benefits when necessary for career advancement.

  Job Retention Services:

Job retention services may include but are not limited to the following:

  Plan Development - Assistance in development and implementation of the post employment plan to address job retention issues as appropriate.

Direct Contact  - CRP will engage in face-to-face contact with individual, family, or employer as appropriate to discuss employment issues.

Case Management - CRP will assist individual with issues that jeopardize continued employment such as transportation, childcare, community resources, or other relevant services.

Job Reemployment  - Job reemployment service will include those services needed to ensure maintenance of continuous employment in terms of the individual’s abilities and interests for 9-12 months at SGA.

  Procedures and Restrictions

 

·        This is a separate and distinct service. It is not an extension of community based work adjustment services currently offered by CRPs.  Individuals receiving this service may or may not receive any other services from the CRP. 

 ·        As with all services, this service must be jointly agreed upon by the counselor, consumer and service provider and an authorization must be made prior to start of service delivery.

·        This service may be provided in post-employment as long as this is the only service the individual is receiving.

 ·        This service is targeted toward individuals who need assistance sustaining employment and utilizing SSA work incentives.  Possible candidates are SSI/SSDI recipients who have a long history of short term employment; who have had numerous VR cases which resulted in employment that was not maintained for nine months or more; who have worked near SGA for an extended period of time but are fearful of losing SSI/SSDI benefits or Medicaid coverage; who are nearing the end of a PASS plan and need assistance transitioning to other work incentives to maintain medical coverage, who have limitations from their disabilities which might make it difficult for them to negotiate the SSA process for work incentives, reporting income, etc. (i.e. anxiety disorder, learning disorder, etc.)

·        Since individuals may be referred after working a period of time, the fee will only be paid if an individual works nine out of twelve months from the time of authorization for the service.

BONUS PAYMENTS FOR COMMUNITY REHABILITATION PROGRAMS (CRP) OUTCOMES MEETING QUALITY OUTCOME CRITERIA

A Bonus Payment of $1,000.00 per consumer may be awarded to a Community Rehabilitation Program (CRP) for outcomes meeting a specific targeted wage.  The consumer must work a minimum of 20 hours per week at or above the average hourly wage for all consumers of the department employed in competitive employment during the previous fiscal year.  The targeted wage for FY 2004 is $9.31.

 A Bonus Payment of $500.00 per consumer may be awarded to a Community Rehabilitation Program (CRP) for outcomes with employer paid health insurance.

 The Bonus Fees are payable only to a Community Rehabilitation Program.

These fees were developed to increase the quality of employment outcomes for individuals receiving Community Based Work Adjustment leading to a Competitive Employment outcome (CBWA) or Supported Employment (SE) services from a CRP and to improve the Department’s indicator related to quality employment outcomes.  The fees are bonus payments that the CRP will automatically be eligible for if the established criteria are met.

  CRITERIA, PROCEDURES, AND RESTRICTIONS

A)    The consumer must receive either Community Based Work Adjustment Leading to a Competitive Employment outcome or Supported Employment services from the approved CRP.

B)    The CRP may be eligible for one or both bonus payments.

C)    Bonus payment will be determined at the time that the standard for payment is met:  after 90 days of successful employment for CBWA or after 60 days of employment for supported employment regardless of when the case is closed. 

D)    The CRP must submit adequate documentation that the criterion/criteria have been met along with the bill requesting bonus payment(s) in addition to the established outcome fee.  Possible sources of verification might be copies of the consumer’s last two pay stubs or copies of the individual’s valid health insurance card from a major insurer.

E)    Once the bill is received and the criterion/criteria has been verified, the original authorization should be adjusted to reflect the increased fee payment. 

F)     Bonus payments will come out of the CRP budget.

G)    The maximum payment for the authorized CRP service will not exceed the established outcome fee plus $1,500.00.

H)    The CRP will be provided with the target amount within 30 days of the close of the fiscal year.

RE: Fee Schedule for Community Rehabilitation program (CRP)

Employment Solutions in Lexington is offering a new service which involves an additional component of skills training with a competitive employment outcome. Training in three skill areas is being offered: Automated Office Systems, Accounting/Bookkeeping, and Building Maintenance.

The fee for this CRP service is as follows:

36I Skills Training with Competitive Employment Outcome $5,000

Referrals may be made to the program immediately. DVR coverage of the program may be provided for a consumer who has already entered but has not completed the program by January 1, 1998.

The normal procedure for issuing an authorization and providing payment for a Community Rehabilitation Program service is to be followed. Should a competitive employment outcome not be realized, the authorization is to be canceled in its entirety.

As you know, the employment duration criteria for closing cases rehabilitated has changed as the result of the new Federal Regulations. CRP Outcome Based Fees for regular cases were designed to meet the same employment requirements and thus assure a successful closure at the time the CRP was paid for the service. For the next few months, you will be closing some cases after 60 days of employment and others will require 90 days of employment. For those regular cases (excluding supported employment cases), the authorization and fee for the cases requiring 60 days of employment will remain $3,200 for a competitive employment outcome (36G) and $1,600 for an extended employment outcome (36D). The authorization and fee for this requiring 90 days will be $3,350 (36G) and will remain $1,600 for an extended employment (36D). Fee for other services for regular cases, 10H, 10P, and 36B will not change.

For supported employment cases, the time required for a successful employment outcome by the provider (60) days was not tied directly to the employment duration required for 26 closure. It has been decided to keep individual placement (35H) to $3,350. This new fee is effective for all cases where service was completed by the provider April 1, 1997 or thereafter. All other supported employment fees and requirements will not change.
