RE:             Fees for Job Placement Services for Job Ready Consumers

                    from Private Vendors

Job Placements in Competitive Labor Market for Job-Ready Consumers from Private For Profit Company or Non-Profit Agency with the Expertise and Capability to Deliver Specific, Performance Based, Employment Related Services

Competitive Employment is defined as:

        In Conformance with the Individual Plan of Employment

        Not Less Than Minimum Wage

        Integrated Setting

        Permanent, Not Temporary, Position

JOB SEEKING SKILLS

Outcome: A consumer who has enhanced employment possibilities.

Services: The services provided by the vendor will include an assessment of the individual’s qualifications for employment and interests (e.g., job type; salary; location, etc.). If a consumer requires assistance in developing a resume or in learning interview skills in order to enhance employment possibilities, the vendor must provide this service. The vendor’s assistance may include group and/or individual coaching of a consumer to obtain employment.

Required Documentation: Results of assessment; resume; documentation of classroom or individual instruction with consumer; and signed verification of consumer that services have been delivered.

JOB DEVELOPMENT/JOB PLACEMENT

Outcome: Consumer is working successfully in a job that matches his/her vocational goal.

Service: Services may include one or combination of the following: initiating and maintaining employer contacts, developing necessary labor market information, identifying specific job openings, conducting job analysis, matching consumer to specific jobs, and preparing consumer for job placement in a timely and systematic manner. Job placement is the process of locating and securing a permanent job in the competitive labor market.

Payment: First payment will be made after the consumer’s fifth successful day of employment.

Required Documentation: Verification of employment from employer; verification of satisfaction from consumer; and information regarding job title, rate of pay, hours, and benefits.

SUCCESSFUL JOB RETENTION

Outcome: Consumer has worked successfully for a minimum of ninety (90) days and meets DFB/DVR criteria for 26 closure.

Services: Vendor has provided follow-up services to ensure appropriate job adjustment and employer satisfaction. Services may include assisting with reasonable accommodations, providing ongoing employer/employee follow-up, and assisting consumer to maintain employment.

Payment: Second Payment will be made at ninety (90) days retention.

Required Documentation: Verification of employment from employer; verification of satisfaction from consumer; and any changes in job title, rate of pay, hours, or benefits.

FEES

Recommended Fee: $1,500.00

First Payment: $ 375.00 (25% of the Total Fee)

Second Payment: $1,125.00 (75% of the Total Fee)

The First Payment constitutes payment in full for all services delivered during Job Seeking Skills, Job Development, and Job Placement Phases. The Second Payment constitutes payment in full for all job retention services provided throughout the first ninety (90) days of employment. Each payment can be made only once per case.
